40 year female, Farmer, presents with: breathlessness since 2-3 days, chest pain since 1 day (acute onset, severe retrosternal pain radiating to back, associated with sweating), fever since 1 day, no cough, rhinorrhea, orthopnea, palpitations, no h/o Pedal edema or oliguria, no h/o headache.
